- Review the “General Information for All Clubs” section on the Membership Information page
- Space is limited, so call or e-mail the Club Director to discuss availability and schedule an appointment to register
- What you need to register your child
- Make an appointment with your specific Club & make sure to bring with you the following:
1. Membership application for each child & all forms attached to the application
2. Proof of income for the entire household (4 weeks pay stub, W2 or 1040 tax for from 2021)
3. shot records
4. social security number
5. Registration fee (varies) - We will NOT be able to register your child if you do not have EVERYTHING that is listed above when coming to your appointment
